A rare medical success in treating a late arterial leak 10 years after aortic aneurysm repair.

The Vascular Surgery and Minimally Invasive Surgery team at EGYPTAIR Hospital successfully treated a complex and rare case of Type 1B endovascular aneurysm (Type 1B endoleaks) that appeared 10 years after a TEVAR (Terminal Endoscopy) procedure for a 61-year-old patient suffering from hypertension. After the onset of symptoms, the case was accurately diagnosed using CT angiography. An intravascular ultrasound (IVUS) catheterization was then performed to precisely locate the leak, located above the celiac artery. A new 38×30×20 mm Ankura™ stent (LifeTech Scientific) was inserted via the left femoral artery without the need for an insertion sheath and was accurately secured. Final imaging revealed complete success with the leak resolved, while the vital arteries remained completely intact. The access points from both femoral arteries were successfully closed using two ProGlide® devices on each side (Abbott Vascular). This case underscores the importance of long-term follow-up after aortic surgery, as well as the power of teamwork and interdisciplinary integration in managing delicate and complex cases.
